The following positions within Inpatient Adolescent Psychiatry, Inpatient Adult Psychiatry, Psychiatric Emergency Services, CARES, and Outpatient Behavioral Health Services are offering the opportunity to enroll in a new program offering a cash incentive to recognize your work treating patients with substance use disorders. This program is sponsored through the ACCESS (Advancing Careers, Competencies, and Equity in Substance Treatment Services) grant. Employees must be >0.6 FTE to be eligible. 50% of the bonus will be received 90 days after signing the program contract.

  • Registered Nurses (RNs) may earn up to $7,500 by December 2024
  • Behavioral Health Technicians (BHTs) may earn up to $3,500 by December 2024
  • Care Navigators may earn up to $3,500 by December 2024

Job Summary

Under general supervision, provide patient navigation services as it relates to care coordination, referral management, and service continuity including but not limited to assisting with navigating the health care system, re-engaging patient care and Patient Centered Medical Home (PCMH) connection, specialty specific health education, information, support, and access to services/resources for all patients.

Essential Functions:

Care Coordination, Referral Management, and Collaboration * Acts as an 'extender' for the specialty clinics by coordinating, supporting, and assisting as needed to facilitate care coordination and individual plans of care. * Assist patients in problem solving potential issues related to the health care system, financial or social barriers (e.g. request interpreters as appropriate, transportation services or prescription assistance). * Acts as the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns. May assume advocate role on the patient's behalf with the carrier to ensure approval of the necessary supplies/services for the patient in a timely fashion. * Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providers. * Using motivational interviewing and teach back tools for reviewing patient instructions * Connecting care within the community (i.e., support groups) * Coordinating amongst providers in an effort to support a multidisciplinary team to enhance patient experience and satisfaction * Serves as a liaison for internal and external care providers and/or families/caregivers * Identifies opportunities and collaborates with specialty care team to implement solutions * Actively advocates for patients to improve their healthcare experience and individual health outcomes. (45%) Educates, Advocates, and Assists Care Navigator Staff: * Assists Care Navigator staff in an effort to improve staff cohesion, compliance with regulatory bodies, improve consistency of standard work, and achieve high patient satisfaction * Collaborates on standard work and deliverables in order to monitor individual care navigator performance * Assists with monitoring current standard work and implementation of new standard work to maintain compliance and achieve a high level of productivity Provides content related expertise to multidisciplinary team as it relates to care navigator duties and responsibilities * Attends and participates in team, staff, and/or other meetings as needed * Acts as an advocate for a "territory" of clinics and their respective care navigator staff in a manner that promotes continuous quality improvement * Collaborates wi h a multidisciplinary team in the creation of training programs, materials, and content in an effort to support system wide navigation * Actively promotes quality improvement surrounding patient care and healthcare services * Works closely with care navigator team to monitor performance, which may include but is not limited to: coaching, mentoring, and motivating to ensure care navigators are providing optimal patient care services and meeting and/or exceeding all department goals * Ensures all grievances are handled with the required deadline to support care navigator staff * Responsible for representing care navigators in various cross functional meetings and committees as they apply. * Preform regular quality control for care navigator duties * Mentors care navigators to achieve success in meeting quality, productivity, and departmental goals through coaching and collaboration. * Willing and able to pursue personal and professional opportunities to enhance leadership skills such as becoming a trainer for the Care Navigator Collaborative * Comfortable speaking to large groups of individuals.
