Overview

The role of the Employee Assistance Program Case Manager entails delivering intake screening, assessment, consultation, and solution-focused support services to all EAP members from diverse backgrounds and populations. This includes conducting clinical screening and assessment in various areas such as mental health, couples/family dynamics, depression/anxiety, trauma, suicide prevention, interpersonal violence, children to adults, abuse/neglect, grief, crisis intervention, advocacy, and substance abuse. Additionally, the Case Manager offers in-the-moment support and crisis stabilization to high-risk clients, as well as handling case management, triage, and referral coordination with affiliate providers within our national network. Direct patient care is also within their scope of responsibilities, requiring the demonstration of competencies specific to the served population.

 

Qualifications:

  • Master's degree in social work, counseling, psychology, or related field required
  • Licensed Master's Social Worker--Clinical, or Licensed Professional Counselor, Limited License Psychologist or Temporary Limited License Psychologist credentials required (must be in good standing and valid in Michigan).
  • Individuals with EAP experience preferred.
  • 3 years full-time professional level work experience in mental health with all age groups, with preference for individuals with a background in treating substance use disorders.
  • Proficiency in maintaining confidential records, developing clinical care plans, and coordinating case management services.
  • Strong communication skills and proficiency in Microsoft Word, Excel, and EMR systems.
  • Willingness to work between Battle Creek and Kalamazoo HelpNet sites.

     

Responsibilities:

Assists clients in scheduling counseling appointments within the national network of providers, coordinating with insurance benefits.

Maintain confidentiality by managing records and files for Bronson HelpNet offices, including highly sensitive materials related to contracts and client activities.

Collaborate with affiliate providers to develop and review individualized clinical care plans and case management services for management referral clients, addressing presenting issues.

Conduct clinical assessments with clients and/or eligible members to determine appropriate levels of care and interventions for treatment and discharge planning.

Collaborate on provider relations responsibilities with the Information Specialist, Intake Team, Leadership, and credentialing agency, including managing provider agreements, development, network coverage, documentation, quality of work, communication, provider engagement, and affiliate referral processes.

Communicate and coordinate services with affiliate providers and corporate clients while adhering to HIPAA regulations and guidelines.

Participate in ongoing education and training to fulfill professional and program requirements, including attending in-services, staff meetings, committees, and completing required competencies.

Represent HelpNet and Bronson in the community through various activities such as speaking engagements, networking, and educating about program services.

May provide minimal psychotherapeutic interventions across various modalities, including individual, group, couples, and families.
