Trinity Health Social Care Manager in Atlanta, Georgia

Position Purpose: Mercy Care Atlanta is seeking a dynamic Manager of Social Care to lead the development, implementation, and enhancement of our Social Care programs. In this pivotal role, you will oversee wrap-around services, case management, and specialized programs to address the social determinants of health, fostering lasting health improvements for our patients.

What You Will Do:

  • Develop, implement, and evaluate Social Care programs to meet patient needs effectively.

  • Lead a team of dedicated professionals, providing supervision, training, and support.

  • Collaborate with senior leadership to establish quality metrics and ensure adherence to standards.

  • Cultivate partnerships with external organizations to expand support for Mercy Care patients.

  • Drive program development, grant writing, and grant management initiatives.

  • Establish and maintain relationships with community resources to enhance service delivery.

  • Champion racial and cultural sensitivity within the organization, serving as a positive role model.

Minimum Qualifications:

  • Master's degree in social work, counseling, psychology, sociology, or related field.

  • Seven years of experience in human services, with five years in program development and leadership roles.

  • Proven track record in multidisciplinary team environments and grant management.

  • Valid Georgia Driver's License (may be required).

  • Strong organizational, management, and leadership skills.

  • Ability to develop and maintain community relationships.

  • Proficiency in Microsoft Office and financial analysis.
